Note:
1. All FMNP Instructors and Assistant Instructors must be on file with the FMNP Conference Coordinator. (All instructor names will be verified. If not on list, name will not be posted online.)
2. Each course is limited to 20 students. However, Instructors are allowed to increase the total up to 22.
3. All registration is in advance.
4. Instructors are responsible for local publicity of their courses.
5. Instructors must fill out the student record and and invoice forms and mail it along with signed liability forms, completed evaluations and unused patches, pins & workbooks to the FMNP Conference Coordinator. Once received, the Incentive fee will be submitted to UF Finance for payment processing.
6. A master file will be kept with the Instructor's approved affiliated organization and incentive payment information. Incentive payment will be made out to the Instructor's affiliated organization, which must be in the UF vendor system.
